- Use this form to request the waiver of a prerequisite for a course designated with a SYSC prefix.
- Course Overload Application Form (online)
- Use this form if you are requesting a course overload and you are a Communications, Computer Systems, Software or Biomedical and Electrical Engineering student.
- Audit Discrepancy Report Form (online)
- Use this form to report discrepancies/errors in your student audit.
- 1st Year Hold (online)
- Use this form to propose a schedule of courses for students who are on "1st Year Hold".
